Cases of alleged breaches by elected and co-opted members of an authority’s statutory code of conduct are referred, by the Public Services Ombudsman for Wales to the Adjudication Panel for Wales, for determination.

Our form to reply to a notice of reference, can be downloaded by using the links below. The form will also be sent in the post to the relevant member. Completed forms need to be returned to the Adjudication Panel for Wales.

  • Reply to a Notice of Reference, the Adjudication Panel for Wales will give written notice of a reference and send with the notice a copy of the report produced by the Ombudsman to the member who is the subject of the alleged breaches. There is a statutory duty on the member to provide the Adjudication Panel for Wales with a written reply and the information set out in our form.
